• Office Hours are 9 – 5:00 Monday – Friday
  • Appointments for the day are confirmed the day before or the morning of the appointment
  • Appointment scheduling routines should be familiar to all office personnel
    New patient appointments are 45 minutes, patients are asked to bring their insurance card, picture identification, list of current prescription medications.
    Self-pay patients are told the fee of $275.00.
    Self-pay follow-up appointments are $125.00
  • Appointments are booked between 1:00 PM – 4:45 PM
  • Outpatient diagnostic testing is scheduled with facilities indicated by the physician  This should be arranged before the patient leaves the office
  • Laboratory work and vaccines are scheduled between 9:00 AM – 11:30 AM
  •  Lunch Time
    Phones are put on the voice mail service between 11:45 AM – 12:15 PM
  • Phones are put on voice mail at 5:00 PM
  • Physician afternoon off:
    Tuesday – Dr Levine and Dr Rodriguez and Dr Perdomo
    Wednesday – Dr Jacobson, Dr. Gaviria, Dr Pena Gonzalez
    Thursday – Drs Baker
  •  Complaints
    Patient complaints of any type should be directed to the Office Manager
  • Emergencies
    If anyone in the office appears to be in acute distress, call 911
  • Advice to patients over the phone
    Only doctors give medical advice to patients over the telephone, unless employee is directed to do so by and for the physician
    If patient conveys a life threatening situation, advise him or her to call 911 or immediately go to the nearest emergency room
